What is SLS?
Supported Living Services (SLS) assists persons with disabilities to live in a home of their own. Various services and supports are provided to meet individual needs for health, safety, and lifestyle choices.

S.L.S. FACTS:Supported Living Services (S.L.S..) is a program that provides an array of services for adults with developmental disabilities who want to live in a home of his or her choice.
S.L.S. assists consumers to identify their individual needs, and provides the support needed to live safely in the their own home and the community of their choice.
S.L.S. is available to anyone over the age of 18, regardless of the nature or severity of the person's disability.
S.L.S. supports and services are patterned differently for each person, based on that person's individual needs.
S.L.S. values the choices, needs, and satisfaction of people with disabilities.
S.L.S. emphasizes the importance of interdependence, building independence while maintaining mutually beneficial relationships or a circle of support to improve quality of life.
DESCRIPTION OF SERVICESINDIVIDUAL NEEDS ASSESSMENT:The Case Manager conducts a thorough and comprehensive assessment to determine the physical, mental, social, and emotional needs of the individual to live safely and happily in his or her own home within the chosen community. During this time the Case Manager establishes a trust relationship with the consumer, learning the persons unique needs and life goals.
INDIVIDUALIZED SUPPORT PLANNING:
The Case Manager, the consumer, and all other persons representing the consumer's best interest, develop an individualized plan of support (I.S.P.) based on the results of the Assessment. The I.S.P. is a detailed plan entailing the S.L.S. provider responsibilities and the consumer's plan to meet his or her short and long term goals in Supported Living. The following areas are addressed in the I.S. P. :, Support Services, Personal Assistance, Training and Habilitation, Assistive Technology, and Emergency Response System.
ESTABLISHMENT OF HOUSING:
The Case Manager assists the consumer in finding and securing an independent home environment which is both safe and acceptable to the consumer. The Case Manager acts as an advocate and liaison between the consumer and the landlord, facilitating acceptance into the property and good relations during the consumer's stay in that home.
ADVOCACY & COORDINATION OF SERVICES:
The S.L.S. networks to initiate all generic services, such as In Home Support Services (IHSS), transportation services, day and work programs, medical, mental health and adaptive equipment services needed by the consumer. The Case Manager and Lead Personal Assistant perform case management, coordinating all services to ensure that the consumer receives the full benefit of each service.
RECRUITMENT AND MANAGEMENT OF STAFF:
In-home Personal Assistant staff are recruited, screened, hired and trained to perform according to S.L.S philosophy. Consumer's also interview potential workers and have the final say as to who will work for them. Once a worker is chosen, that worker is trained to provide for the consumer's individual needs and wishes. The S.L.S. team then frequently monitors, supervises, and gives on-going training and support to the S.L.S. staff and IHSS workers.
PERSONAL SUPPORT & ASSISTANCE:
Personal assistance is provided in the home and community setting. This service includes support and assistance with the activities of daily living, such as help with bathing and hygiene, assistance with taking medications, meal preparation and nutrition, housekeeping, money management, community access and development of a community oriented life, etc.
TRAINING & HABILITATION:
The S.L.S. provider provides instruction and support to help develop basic living skills, such as budgeting money, shopping, cooking, personal hygiene and grooming, and social skills, etc.
DEVELOPMENT OF CIRCLE OF SUPPORT:
In addition the paid services, "natural supports", such as family, friends, neighbors, school peers, and co-workers are a vital resource for consumer's in S.L.S. A circle of support is developed and maintained to provide a large network of support to help ensure safety and quality, meaningful life.
24-HOUR CRISIS INTERVENTION:
A trained professional is available for emergency assistance day or night.
